Transitions Showcase Event

Since 2014 the Transition team have been working with young people between of 18 - 37 years to support them with their transition from CHAS to age appropriate services. At this event young people will share their experiences and achievements. There will be a chance to meet with service professonals and find out more information about whats going on in your area.

Refreshments and lunch will be served during the day. can you please advise us if you have any dietary requirements.

services & organisations who work to support people with disabilities.
