CEL 6 (2008): NHS Continuing Healthcare
This letter provides revised guidance on NHS continuing health care and replaces the previous guidance contained in MEL (1006) 22. The previous guidance made specific reference to people who were near the end of life and included a section on palliative care. The revised guidance makes no distinction between any client groups. It states that those whose prognosis is that they are likely to die in the near future are entitled to a range of services to meet their needs in the same way as anyone else. It also makes reference to the SPPC report Palliative and end of life care in Scotland: the case for a cohesive approach, and its recommendation for future guidance to more clearly emphasise joint responsibility for planning and financing palliative care.

The future care of older people in Scotland

This document sets out a vision for care with the emphasis on helping older people to stay at home; enabling people to be cared for in the least intensive setting possible; and a whole systems approach. One of its recommendations is that: ‘consideration should be given to additional investment in improving palliative care services in Scotland, building on the success of the Gold Standards Framework and of the NHS End of Life Care Programme in England, and the effectiveness of such end of life care initiatives in Scotland should be evaluated.’

A National Framework for Service Change in the NHS Scotland (AKA “The Kerr Report”)

Partnership for Care: Scotland’s Health White Paper

Joint Future is the lead policy on joint working between local authorities and the NHS in community care. Its main aim is to provide faster access to better and more joined up services through improved joint working. Implementation began formally in April 2003 with delivery of the key elements for older people's services. They were extended to all community care groups from April 2004. Single Shared Assessments (SSA) were introduced as part of the Joint Future initiative, and aimed to eliminate duplication in assessment and ensure that information is shared across agencies.

This document emphasises the importance of service level agreements and shared service planning between health and social work, to provide a co-ordinated strategy for the organisation and provision of care. It also sets out the responsibilities of NHS Boards with regard to palliative care provision, including funding and patient-choice. (Now revised under CEL 6 (2008))
