Partnership publications

This report highlights the role of the Community Hospital in providing palliative care as a local resource close to the patient's own home and in enabling continuity of care from the local family doctor and primary health care team. It provides guidance for those responsible for the planning and delivery of palliative care in community hospitals, and for those who work with them in related services.
