Introduction & Purpose

To improve our knowledge of current practice and use of Home Health Monitoring (HHM) by health and social care services across Scotland, the Scottish Centre for Telehealth & Telecare (SCTT) has designed this short base line survey and asks for your assistance.

The information gathered from this survey will be used to inform the development and expansion of Home Health Monitoring in Scotland and final results will be shared with all participants and through the Improvement Support aspect of the Scottish Governments: Technology Enabled Programme, Delivering our Ambitions.

Telehealth

Telehealth, or similar terms such as “Home Health Monitoring“ (HHM), Remote Patient Monitoring (RPM), and Telemonitoring enables individuals to take more control over their own health. It can become an important part of an individual’s care by enabling physiological information about their health condition to be monitored regularly to flag immediate issues anchanging/deteriorating health status. It has a role to play in enabling people, such as those living with Long Term Conditions, to stay at home and in their communities for as long as appropriate.

Within HHM, patients use digital technologies to collect their own health data in one location and electronically transfer that information securely to an appropriate professional in a different location. The parameters measured may include, for example, blood glucose, pulse oximetry, weight or blood pressure. This “vital sign” data is accessible by health and/or care staff that, together with the patient, will use the information to better plan their care and monitor their condition. There is a wide range of different technology combinations available which support the delivery of monitoring services .
